Copy of Claim intimation given to Company together with xerox of policy & premium receipt Ear Tag Death Certificate Post mortum report Certificate given by any two of the following :- i) Sarpanch of the village ii) President or any other officer of co-op. credit society iii) Official of the milk collection centre. iv) Supervisor/Inspector/Officer of any Banking or Credit institution (other than the financial bank). v) DRDA or its authorised nominee. vi) Village revenue officer/village accountant. vii) Head Master of primary school.
